Risk Assessment

How to complete the Risk Assessment Forms

1. Project Risk Assessment
This form should be completed by the Principal Investigator initiating the project. The assessment is normally valid for the length of the project but must be reviewed annually and supplemented whenever there is a significant change in the nature of the work to be performed.

2. Experiment Risk Assessment

This form must be completed jointly by any undergraduate, graduate, postdoctoral worker or visitor and the Principal Investigator for any procedure/system of work carried out before an attempt is made at the procedure/system of work. The form should be kept by the research worker. For any one project, a number of such Experiment Risk Assessments should be prepared or referenced from earlier prepared files and combined into a Risk Assessment Dossier for a particular Project. The Risk Assessment Dossier will contain the Project Risk Assessment and the associated Experiment Risk Assessments and can be referred to by a new student/research worker working on the project. If the project involves any CMAC facilities, risk assessment for analysis of the compounds under the project should be done and a copy given to CMAC facility for approval. Use of CMAC facilities for analysis of samples for or on behalf of non-approved projects is strictly prohibited.

3. Teaching Lab - Experiment Risk Assessment

This form is the same as the Experiment Risk Assessment but for teaching labs. The form should be filled in by the lab technologists of that lab together with the lecture-in-charge of the module. The forms should be kept in a hardcopy in the lab and should be made available to any student of the module upon request.

4. Teaching Lab Staff Risk Assessment

This form should be completed jointly by any member of the Teaching Laboratory Staff and the Lecturer-in-Charge. The Assessment Record is valid for a maximum of one year but must also be reviewed and supplemented whenever there is a significant change in the nature of the work performed. There are a number of prepared risk assessments for normal laboratory activities that the lab staff should view and be informed. Viewing of the safety CDROMs on the different safety aspects constitute a form of training.

5. Service Lab Staff Risk Assessment
This form is should be completed jointly by any Laboratory Staff Member and the appropriate Lab Manager. The Assessment Record is valid for a maximum of one year but must also be reviewed and supplemented whenever there is a significant change in the nature of the work performed. A brief description of the work involved and the associated hazards should be given. Prepared risk assessments and safety CDROMs relevant to the field of work should be viewed.

6. Undergraduate Risk Assessment

The undergraduate risk assessment form should be completed by undergraduate working in Research Labs. The student should view the prepared risk assessments and verify that he/she has done so. Undergraduate working in the teaching lab should view the Safety Information sheet and the Undergraduate Safety note, signed and return the acknowledgement receipt to the teaching lab where he/she is doing the experiment.

7. Risk Assessment Non-ChemDept Analysis

This form should be completed by anyone requiring the services of CMAC. Analysis can only be done after approval by lab manager.
